Steps to follow:
- Let us know that you plan to submit your bill to your health insurance for reimbursement. We will email you a Superbill for today’s visit. It includes the NPI number, Tax ID number, amount paid, Diagnosis codes and CPT (service codes) the insurance will require you to submit.
- Contact your insurance plan to see how to submit this information to them. Often, they have a specific form that they will need to provide you with for your specific plan.
